Omg .. I feel for your wife because that is one of the worst things I ever encountered. The pain from an anal fissure is sooo painful. Let me ask, does she do sitz baths? Does she use wipes to wipe with? Does she do squeeze exercises in her anal area to promote blood flow for healing? I also have a cream I used, which is called calmoseptine. This was a life saver. You can order it off the internet. Whenever my daughter or I have a problem back there that's what we use. I did sitz baths whenever I was hurting really bad , especially after having a bowel movement. I hope these suggestions help.
