Try to look into vestibular headaches. That's what my PT is saying now - the dizziness & nausea could be the key.

Are you doing vestibular therapy specifically? My PT has been working on my neck and back since my fusion in March 2022, and also muscle strengthening since January 2023 when all my other symptoms became debilitating. The doctor I saw yesterday suggested aquatic therapy as well as vestibular and balance therapy.
