Struggling with making an osteoporosis treatment decision
I am just struggling to make a decision about my treatment. I had my first bone density test and found I have osteoporosis. I have a couple of -3.2 vertebrae. My hip bones less problematic. I'm 60, active and fit. I've seen an endocrinologist and a rheumatologist who were both highly recommended. Both said "Evenity" before I barely sat down. But there is so little known about Evenity, and nothing known about its long term effectiveness or risk. I've read heart breaking posts from women who were advised to take Prolia with the same assurance and then had multiple debilitating fractures because so little was known/admitted about rebound risk. I am tearful and anxious and sleepless. I've been so healthy my body has carried me through so much life and adventure. I just don't know what to do , whether I'm putting me/my body at risk. Both doctors are paid consultants for Amgen. I feel hopeless and distressed. One of the doctors, although I said I wanted to consider my options, went ahead and got pre authorization for Evenity from my insurance "to show me how easy it would be". I feel cornered. My general doctor also has concerns about me being put on a relatively unknown drug when I haven't tried something like Forteo with a long track record.

One caveat on the D3, which you should check with doc. I was told that Tymlos (so maybe Forteo?) "eats" D3. I have taken more this winter than before Tymlos, meaning 2-3,000. I am going back to 1,000 soon. My level was 52 at the last check.
@yani2023 glad you have an endo who knows kidney transplant issues.
Teriparatide is Forteo. Both Forteo and Tymlos work through the parathyroid pathway. I chose Tymlos due to the adjustable dose.
My understanding is that Teriparatide goes through the kidneys. Prolia does not. I am on Prolia. (3 injections) This last injection I had a little muscle pain in my left shoulder for a few days after the injection but I don't know if that was from the Prolia or weights. After one year of Prolia, I gained .5 in my spine! (-4.1 to -3.6). I have two neighbors on Prolia that have no side effects. They have not had such remarkable results. I eat prunes, take collagen and get all my calcium from food and milk. I also take 200 mcg of K2 Mk7.
@pami and @yani2023 can you share info on the effects of Forteo (and presumably Tymlos as well) on kidneys? The only info I can find is on raised calcium levels, which can cause kidney stones.
The problem with Prolia is what happens when you stop. My doc won't prescribe it.
Windy Shores, I am not on Prolia at of this date. Still trying to find an endocrinologist who has treated kidney transplant patients with medication including biophosphates. There is just so much information to understand. I would like to find other people in same situation perhaps out of the US. Many factors to consider. Other medical conditions a person may have which has to be considered and is very important when being prescribed medication for osteoporosis. I believe this is the reason that Prolia and other medication for bones have different results with individuals who post. My belief only is that people with osteo just want a medication that is safe. My husband had a client who was on Prolia for over 8 years and she still broke her back, ankle a and had minimal improvement. In my case I have to find other kidney transplant individuals who were prescribed a that improved spine health. It's necessary to learn of their experience. If you are a researcher, one can see that much of the studies are quite old. This is a bit depressing.
I am not convince there is a perfect medication for osteo... Research is costly and in these times participants may not be forthcoming to test particular medication for osteoporosis.
I hope all goes well with you taking Prolia. Best of Luck to you... By chance did doctor discuss how long your therapy with Prolia would be? Months or years? Which medication is prescribed after Prolia is stop if your numbers improve? Thanks
I did this test today but he moved it to Black Mountain NC
I'm 75 and struggling with the same decision. I've cancelled my first evenity injection and may do it again until I feel any sense of certainty. I have been on forteo and bisphosphonates. Currently on a break. Meanwhile I have started Osteostrong (would love to get feedback) and starting my second month. Also considering going to Europe and having MBST treatment. Heard about this through an OsteoBoston you tube video. Would also like to know if anyone has received MBST.
I forgot to add my concern about feeling pressured to start evenity. However I don't know your history and am not a doctor.
Are you being heard??? If not are you in the right place?
Dear hey hey,
Your Dec 2020 post just showed up on my feed. I, new to this site and have had a few very helpful responses to my questions about about Prolia for my osteoporosis. Like you, I’m in my early 60’s and very fit and active. My lumbar spine dropped from -2.7 to -3.1 and I feel I need to take action with medication. What did you decide to do? And how did it turn out?
I’m fretting major over this decision as well. We only have one body and I want to live actively for hopefully 20 more years!
