Brain MRI Help

hello! i am 20 year old female. I have Chronic migraines, Hashimotos, EDS, Fibromyalgia. when i was 14 i had a major concussion (4 big boys threw me up and i slipped through their hands and fell straight onto my head… cheer)
since then i had to go to physical therapy for 2 years for my brain and 3 years of physical therapy for my eyes. I had an MRI when i had my concussion and everything was cleared. but for 6 years ive had major migraines and i am now able to manage it a bit with medication that works and, every 3 months of Botox and monthly injections. I recently had a change of headaches/ migraines. Its very painful stabbing feeling on my left side of my head that comes multiple times a day that last a few seconds- few minutes. i let my doc know and we did an MRI and these are my findings.
idiopathic intracranial hypertension related to narrow transverse Dural venous sinuses. Small intermedia versus Rathke cleft cyst. Probable small infundibulum or aneurysm along the anterior cerebral artery left A2 segment.
"The sella is partically filled with CSF with concave morphology of the pituitary. Hypoenhancing nodule in the posterior gland measuring 5mm the distal transverse Dural vernous sinuses are hypoplastic, and stenotic, greater on the left."
Can someone explain to me what this all means? its been days and my provider hasn't/ department hasn't called me to explain since she is out of town…

Hi @Kyleew I am so sorry you have gone through so much and waiting is beyond frustrating. In most healthcare systems there is a doctor of the day that covers for colleagues that are out of office etc. Hopeful you may contact medical assistant to request that if available and if not offered may I suggest calling member services/outreach dept to seek your healthcare systems protocol for providing timely information to you. This information can be very complicated yet many times relates to low risk/easily treated issues so best to hear from experienced professionals. Hope this helped and wish you the best!🌈

First, let me start off by saying— I am not a doctor. I work in the medical field with Endocrinology and I’m only translating this, because I know what it’s like to have test results before the doctors call… and Google is not your friend.

So, in a nutshell— there is more pressure inside your skull then there should be because the tubes that normally drain the spinal fluid from your brain to your blood stream, are more narrow than usual. This is usually something that happens while you are growing in your moms belly and you just never knew because you never had pictures taken of your brain.

There is a small fluid filled cyst and a small bulge in the artery caused by a weakened wall. The aneurysm sounds very scary, but this is also a very common thing and, is probably something that you’ve had for a while.

The next part mentions the sella is filled with CFS. The sella is supposed to have some cerebral spinal fluid in it. It can start to cause problems when the seller is too large and presses down on the pituitary gland, flattening it. When the sella does that, called empty sella syndrome, it stops the pituitary gland from producing all the hormones that it needs for your adrenal glands, your liver, your ovaries, and your thyroid. yours is not flattened, but the seller is pushing on it.

The very last part just mentions those narrow tubes again and this says they’re more narrow than typically seen.
The only thing that could potentially be concerning, would be the 5 mm hyperenhancing nodule. Again, I don’t work in neurology and I am not a doctor. I do regularly read similar reports for patient’s thyroids. In the thyroid, if there is something “hyper enhanced” it means it is a hard mass . This could be a growth like a small tumor- just know 5mm is tiny. The majority of tumors are not cancerous. A lot of people have small tumors that are managed by watching it overtime and many of them do not get bigger.

I would absolutely call and ask for a covering doctor to discuss the results with you, so you are not driving yourself insane until your doctor comes back. I know where I work no one else in that facility- Other than a physician- will be able to give you the results if your original doctor did not see and sign the report.

hello thank you for responding!
My current dr who ordered the scan only responded to the hypertension and nothing else, she works in the headache clinic so my guess is that she doesn't know the answers??. i tried to push to be seen so we can talk about what the findings mean and they wouldn't schedule me in.Its been over a month and im finally getting help. i switched to a new dr and she is doing the best she can from the first appointment! She ordered a CT scan as well as a lot of blood testing panels and talked to her colleagues to get some insight for a plan. My main concern right now is the pituitary tumor. i believe this is causing a lot of symptoms. my Chronic stabbing daily headaches, Thyroid issues, and menstrual not regulations. I don't know if its pressing on my gland causing these issues, but in my gut i feel like it is.
For the sella part as you mention is it a CSF leak? for months prior i learned about CSF leaks and the symptoms matched up prefect to me but never did anything about it.
For the infundilum or aneurysm my new dr seems to be a bit concerned on this area due to it being in a not common area for infundilums. Hopefully with the CT scan answers will become more clear!

I am so sorry you are going through this and not getting answers from your doctor. This comment won't be helpful except for allowing me to vent about the state of medical care in our country. What does it say when someone has an MRI that shows many questionable issues and instead of getting answers from the doctor, the patient has to post their questions in a forum such as this? This, along with the fact that getting an appt with to any specialist is usually a 3 month wait at best. So frustrating.
