Getting your first appointment

I’m not sure if this is the right answers place to ask. Husband has a history of cancer in parotid gland (5 yrs) bypass surgery (25 yrs), and on-going COPD (severe). Past winter has struggled with weakness and shortness of breath. Also passed out twice. The past two months he’s seen pulmonologist and lung function test showed no change from last year. Vascular to check his damaged/blocked carotids, also stable and no charge from last year. Now a cardiologist. Echo and stress test showed no blockage. Currently on a heart monitor. We are not getting anywhere with anyone at our current medical center. Where do we even start if we were to change to Mayo? Husband doesn’t want to start the testing process all over again. Please help!!

On this page, you'll find all the information you need to know as well as frequently asked questions. To start, you can either fill in the simple online form or call Mayo Clinic during business hours to schedule a time to talk to an appointment scheduler.

Here's some additional information about filling out the form since it is now the weekend.
- You can fill out the form on your husband's behalf.
- The form is divided into 4 sections:
1. Patient Information: name, contact info, etc.
2. Primary Concern: 300 character limit to describe the main reason you wish to be seen at Mayo Clinic.
3. Insurance Details: optional, not required to submit the form
4. Wrap Up: 300 characters to tell us anything else you'd like Mayo to know.
– You can select a day and time to be called back to talk with the scheduler.
