What is a Behavioral Health Unit really like

I was in a behavioral health unit in Florida. The surrounding was like that of the usual hospital and the rooms were the same. Recently I was in a behavioral health unit in Baltimore that was like a real jail. The beds were on the floor. The call bells were on the wall at the side of the bed. There was a security guard on the floor, everyone got served the same meals and the lights were on in the rooms 24x7. There was an observation room where patients said they were beaten by security guards. Everyone had to take care of themselves. I saw one in Washington DC where the beds were on the floor, call bells were at the head of the bed and everyone was expected to take care of themselves. But not as much as the one I was in. Do only hospitals in the Baltimore Washington DC metro area have beds on the floor? I have never known of a behavioral health unit to have a security guard on the floor.
