Either one, depending on your relationship with each. I happen to have a long-standing relationship with my ortho, who is a huge believer in exercise and PT, so I would go to him. But for many people, I find family docs are often more helpful when working on long-term problems. They are more likely to see you as a "whole person" and not just look at an x-ray and say "the joint looks fine."
