I just had occasion for a GI consult on another issue, and because this issue is a hot topic on Connect I asked why they start with these 2 tests. Her reply was that it is protocol because over 70% of anemia cases show a bleed in one of the 2 tests, even now, so it needs to be addressed. Once that is ruled out, they look for other causes. She said if they give iron or blood transfusions without determining a cause, and the patient feels better, even temporarily, they go away without an accurate diagnosis and may not come back. This can be life threatening.
