Surgery for GERD

Posted by aliceorangebike @aliceorangebike, Dec 18, 2020

I am considering having surgery to resolve my terrible GERD problems. Toupet fundoplication has been recommended for me but it wraps around only 75% of the lower esophageal sphincter (LES). Has anyone had this surgery? Did it resolve your severe GERD problems? Do you still need to take Proton Pump Inhibitor medication? Thanks for your input!

@bevlynn54 .. I have chronic digestive disorders, GERD , Gastritis, Esophagitis, Small hiatal hernia, diverticulosis, and some IBS . The worst is 3000-4000 deep wet horrible belches and non stop churning gut 24/7 .
I’ve seen 5 Gastro Drs in 7 years. Tried everything under the sun . I’m off to Mayo Clinic Jacksonville tomorrow. I have to re do , 2 hr SIBO breath test , Entraogrophy C scan w/ contrast and another endoscopy with endo flip this time . In 2 days of testing . Plus they do a Covid test 1 st day . So 3 days .
Took me 7 months to get televisit. I’m at my last ditch effort.
Cannot eat anything w/ o major suffering. I eat so clean and healthy too .
I’m a mess . I hope you get the answers you need . I pray 🙏 they can figure me out . I live on Ultra Pepto and that’s it for my issues now . Can’t take much more . God Bless you … Rosemary

NF stands for Nissen Funduplication surgery for acid reflux. I have never heard of achalasia type 111 but if I were you, I would be asking a whole lot of questions about the procedure that you are considering having done. All procedures have risks and in my case and may others like me that had the NF done, are suffering more than ever did before. I look briefly into the procedure that you are thinking of and it looks like it is new and doesn't last. It would have to be redone and there are several complications. If you have Face Book, look for groups that are specific to achalasia. I'm sure that there is one. That is where I have found a lot of support and answers to many question to my condition. Best of luck to you!

Thank you for responding. From what I read on all procedures for all three achalasia types; all of them have pitfalls. I'm definitely going to be asking a lot of questions. After waiting already for so long; I don't want to have to do more suffering than I already have been. This condition is debilitating; quality of life isn't good. It's been a wakeup call for sure. I guess not everything can be fixed 100% I am on FB. I will see if there is a group on the subject. Thank you.

I highly recommend the Linx procedure for anyone that has serious acid reflux or gerd. At least it can be reversed if it doesn't work for you. Acid reflux can lead to more serious health problems if not taken care of. Doctors at the Mayo Clinic think that reflux contributed to my husband's pulmonary fibrosis. The acid aspirates into the lungs and causes permanent damage. So, if you have acid reflux, please get it taken care of.

Rosemary,
I hope you find answers you need on your visit.
I was at Mayo Jax a couple of weeks ago. My team got changed. I needed my esophagus dilated again. Solid foods would not go down and liquids were beginning not to.
My new team is Jane Diamond NP and Dr Wolfsen. I only saw Ms. Diamond. I really did not spend enough time with her. Important thing is I am swallowing solid foods again. Scheduling worked hard to get me in on short notice. I did covid test, blood work, ecg and saw Ms Diamond in the morning and had the endoscopy (Dr Buras) in the afternoon. .. A whirlwind day!!

Please update when you can on how your visit went.

Please do not get the Nissen Fundoplication. It was the worst decision of my life. Please try a holistic medicine expert to help you with your diet. There is no turning back if it doesn’t work and there are numerous side effects/complications the surgeons don’t tell you.

What are the side effects/complications the surgeon's don't talk about? I'm scheduled for the Toupet in December.

The biggest complication is if they cut or damage the vagus nerve. Second is that The Toupet fundoplication or 270-degree wrap is equally effective in controlling GERD but tends to fall apart with time due to lack of strong serosal adhesions between the two edges of the wrapped gastric fundus and your GERD will return. Gas and sever bloating is also common and has effected my whole life. There is a facebook group called Nissen Fundoplication Take Down, that have several story's of people that have had complication that they don't tell you about. I have a NF 360 wrap that has not slipped but I'm so bloated that I there is almost no room for food, I can hardly eat any thing. I'm not absorbing vitamins and now I have SIBO. I will be having my NF taken down in Jan. This surgery was the worst decision that I have ever made. Also to top thing off, they never tested me for low acid in my stomach. Low acid in your stomach can cause the same symptoms as GERD. Heart burn and acid reflux!!!! Make sure that they test you and that you really need the surgery in the first place. I wish that I would have know about this 4.5 years ago. I really wish you the best.

Son had fundoplication 21 years ago at 5 years of age. Saved his life, then changed his life.

Hi, I know of people that have very successful Nissan and Toupett Fundoplications. I was also hoping to go off my PPI'S since I had been on them 12 + years and all the side effects.
Bc of my failed Toupett Fundoplication 3 years ago, it has been a nightmare. I developed severe upper regional chest pain since week one. My surgeon was in complete denial that the Toupett was causing it. Admitted to the hospital and went through a complete heart work up. My heart was great! Thank you God!
My surgeon still refused to do a reversal or help in any way.
I was hospitalized 3 more times in attempts to find the pain. My pain was and is so debilitating, Percocet is like taking a Tylenol.
Finally 9 months later I found a wonderful surgeon willing to go in, repair my hiatia hernia and remove scar tissue from old surgeries, hoping that would alleviate the pain. We gave it 2 weeks and he scheduled me for the reversal of the Toupett Fundoplication .
I had relief for about 3 weeks and the pain began to return with a vengeance.
I found out that I had gastroparesis, probably from damage to my vagas nerve.
Began with the GI team over at USF Tampa and went through every test and scan possible.
Also, a G-Poem for my gastroparesis.
A G-Poem is a procedure done on the spinster of the small intestine to widen it by making incisions around the spinster to allow food to go into the small bowel
A Poem is a procedure done on the spinster of the esophagus.
Just wanted to clarify the 2 procedures.
Anyway, I'm almost 4 years down the road and rhe pain in my upper abdominal region has only gotten worse. If I have any upper movement for over 20 minutes, I feel the most debilitating pain, have labored breathing, break.out in a sweat and am bend over the pain is so severe
I was just referred to The Mayo Clinic but their appointments are 3-6 months out. My doctor is looking for another specialty clinic as I type this.
Would I recommend having a Toupett Fundoplication? I guess that decision is alone for you to make.
Good luck and God Bless!

I Have post operative gastropanesis.. the result of a Nissen fun. failure .. that was in 2015.. the damage to the hairnet like nerve system that empty the stomach were damaged... they do not regenerate.. there is no reversal.. every meal is a challenge.. now 85 years old.. I wanted my local surgery group to be skilled but they couldn't even figure out what they did wrong.. mayo Minnesota figured it out in a day and gave me a way to cope..
