My next question is who detetmines the dosages. If you have never taken the medication before how can you determine your own dosage.

The prior responder is correct. The doctor ordering the compounded drug must specify what they want unless the doctor has a predetermined compounded mixture they order frequently. Be cautious about why the doctor feels you cannot be treated with standard medication already available and what kind of proof can they offer showing the benefits of this compounded medication? It might cost you or your insurance a lot of money too.

My guess is a doctor has to provide the prescription but I have no experience with a compounding pharmacy/pharmacist
