Here are results for requesting my deceased mother's medical records records.

I scanned the following documents and sent to MAYOHIMSROI@mayo.edu. You can fax or mail the documents if do not have ability to scan.
- Mayo's Authorization to Release Protected Health Information to a Third Parry - signed
- Death certificate
- Power of Attorney signed prior to her death
- State of MN Probate document stating I was Personal Representative of her estate.

Not sure if needed all of them, but decided it was better to send to much then not enough.

Took a month because some if it was at off site storage, but today I received a package with her medical records going back to when she had molars pulled as student nurse in 1951.

I found the info on eye surgery that I needed.

I will now put the stack of paper in box and store incase myself or siblings ever need medical information for their medical care. I have no desire to read all the information, I want to respect her privacy even though she is gone.

I'm glad you were successful, Laurie. Amazing to be able to get complete records back 70 years. Thank you for following up with the process and contact information for others who may need it in the future.
