The form has been updated and now consists of 4 short sections:
1. Patient Information
This section takes about 5 minutes. You are asked to fill out information like your name, age, and contact information.

2. Primary Concern
This section takes about 2 minutes. You are asked 2 questions:
a. Please tell us about your primary medical concern. This will help us learn about how we can help. In 300 characters (~60 words) or less.
b. Are there any other medical concerns you’d like to discuss? In 300 characters (~60 words) or less

3. Insurance Details

4. Wrap up

No more essay questions. Phew.
