I have CKD and I’m near Stage 4 with eGFR of 34. I developed Gout in September of 2019. My PCP gave me Prednisone for it (20mg for 3 days, 10mg for 3 days, then 5mg for 3 days). He wouldn’t prescribe the other Gout Rxs because my Creatinine Clearance was 51. After the second attack in December 2019, he started me on Allopurinol 100mg daily. After I had a total of 5 attacks within 10 months, he upped my dose of Allopurinol to 200mg. The increased dosage caused my legs and feet to go numb 2 hrs after I took it. My Nephrologist said that Allopurinol was causing Peripheral Neuropathy and that it was not good so he switched me to Uloric (Febuxostat) 40mg a day. That stopped the Gout attacks. It is expensive and BCBS wouldn’t approve a Prior Authorization for it, so here’s what my Nephrologist did: He wrote the Rx for 80mg (which I cut in half) for #90 and gave me a sheet for Costco Pharmacy which said it would cost around $165.00
which would last for 6 months (so it equals less than $30 per month). My local pharmacy agreed to give me the same price as Costco. I haven’t had any side effects from it, and my Uric Acid went from 5.6 (which still caused Gout) down to 2.4.

Thank you so much for your feedback! I am now On Allopurinol, 150 mg. Have not had any gout, but continue to have painful feet. I will check with my neph on this other drug. Interesting and informative and creative prescribing!
