Glad you got the vaccine Merry! I first tried at a Publix grocery store, was online in a waiting queue for 3 hours and finally got through. That form asked if I was allergic to any medications. I checked "yes" and it had me list them. Penicillin & Sulfa. BOOM, I was told they could not administer the vaccine to me. In the meantime my neighbors were going to a Mall and never had to fill out any forms. I was just too nervous to be inside a mall, in a line so choose not to go that way. Then Walmart grocery store opened up for vaccines and I got online and had to fill out a long form. The 4 questions I had to lie about where: 1. have you had cancer 2. have you had chemo or radiation 3. Have you had a skin scrapping in the last 6 months 4. Are you allergic to any meds. Glad it is all behind me....I have one more week to go before I will slowly venture out to a grocery store with my N-95 one!!!

@seaspray- I'm allergic to sulfa too. The only thing that I was asked on the form that I felled out was if I ever had an allergic reaction that was severe (anaphylactic) and I haven't.
