@lindes I should have asked and not assumed as much. From what I saw it looked like it was covered for certain diagnoses. You would think that surgery would show medical necessity but, as you know, it doesn't always work that way.

Well the good doc from the mayo said physical therapy would be a good thing to do. I should have followed up with him for a prescription, but it was the day of being released from the hospital, and I had my head spinning anyway. But I will check back with them.
