There was a Linncare small little office in Bartlesville, Okla -- I just went in there. It was no problem. If you're not close to an office, I would just start with a call and if that's not sufficient I'm sure they can direct you where to go.

Thanks auntnanny. You mention Bartlesville; a place I think of as the "biggest, brightest" little city anywhere. With the unique Price Tower ala Frank Lloyd Wright, Phillips Petroleum, Reda Pump, Frank Phillips' historic home and Woolarock a few miles from town.
