#1 Self referral: If you would like to make an appointment at Mayo Clinic, please call one of our appointment offices. You can also request an appointment online. The contact information for Minnesota, Arizona and Florida can be found here: http://mayocl.in/1mtmR63

#2 Have your physician make the referral: You may speak to your physician to see if they are willing to make the referral on your behalf. This allows them to share any/all lab work and imaging at the time of the referral so your medical information may be routed to the appropriate department for review. I am not sure which area of the country you reside in so I will share the three location 1-800 numbers so you may share the one most appropriate with your physician.

Hey @terrydclaar! Guessing my referring neurologist marked my case as urgent. Other than that, nothing to my knowledge was done. I’ve been on the Mayo waiting list since mid February & was shocked in the best way to hear from them so quickly. I was told that Mayo was alarmed by my age, the unknown cause of this ailment along with my extensive medical treatments. Grateful doesn’t begin to express how thankful I am for even being noticed. I’ll keep you guys updated!
