Bladder neck constriction

Posted by tom1984 @tom1984, Jun 15, 2019

Had bipolar TURP in early April and latest cystoscope showed bladder neck contracture. Dr wants to do dilation but from what I have read this isn’t usually a permanent fix. Any other treatment suggestions or experiences? TURP followed acute urinary retention with several failed trials without catheter.

@tom1984, I'd like to introduce you to a few other members, like @victor1930 @jakedduck1 and @predictable, who may have useful experiences with bladder neck obstruction or urethral stricture, as well as TURP.

Tom, did you have the dilation done in the meantime? How are you doing?

Hi Tom,
Welcome to Connect. I like your username, I like anything that’s easy to remember.
Did you have a prostatectomy or other prostate surgery.
My first recommendation is not to have dilation. It hurts like hell! and isn’t that successful. Of course maybe your not the wuss I am. They can use an anesthetic and if the doctor refuses grab your clothes, run to your car and floor it!!! There is another type dilation procedure where a balloon is used so only the stricture is dilated. It would probably hurt less but I’d still look for an alternative treatment. I’d ask about having a Urethroplasty. Your doctor will probably agree that’s the best Stricture treatment.
Good luck with whatever you decide.

Thanks again for the reply. I opted for a bladder neck incision but the urologist did a urethral dilation as well while I was under general anesthesia. He said it is a waiting game now to see if I scar down again. Flow is already slowing down after only 4 weeks so I’m getting more pessimistic. Will mention the urethroplasty on my follow up visit.

Thankfully you had anesthesia for the dilation but I don’t believe the fat lady sang. I hope you share the reason why your doctor chose against the Urethroplasty.
Good luck,
Jake

I am a 41 yr old Male.

I am supposed to have surgery this week for a tight bladder neck. I have had the issue for 7 years and my bladder is getting weaker.
I am very scared and not sure if it is the right decision. I have been on flowmax and it helps some. I am on two pills totaling .80.
I am 41 years old and had a qmax score of 6 without any meds and 12 with meds. The meds cause some side effects like eczema and some dizzyness, maybe even some back and side pain.

If anyone has an experience with having a BNI at 5 and 7 could you please share with me your experience>?
My BIGGEST fear is incontinence. I am 41 have two kids and a beautiful wife.

Did you get and form of Incontinence?
Did you get the dreaded Retro?
How long was your recovery time?
Would you share with me your outcome and if any complications?
Did you get the results you were looking for?
Would you do it again knowing what you know now?

Am I jumping the gun having this surgery or am I scared of things that are very unlikely?

I see some people have good flow after the surgery but then find out a few weeks later their slow flow comes back. Thank you for anything you can share.

Thank you,
Joel P

Hi Joel,
I had BNI at 5 and 7. I did not have any incontinence problems. However, I had a lot of bleeding after a bowel movement a couple of days later. Maybe because the rectum is at 6 o'clock. I had to go to the ER to clear the obstruction due to the blood clots.
After this issue I was urinating great. I am at 5 weeks and I starter to see the flow slow. I am getting worried and thinking what I should do next.

Thanks,
jcesarf

@jcesarf, did you report the slow down of your flow to your doctor and/or surgeon?

Yes. He said to wait for things to settle. I am at 6 weeks now from when the BNI was done. My appointment is at 3 months. Hopefully things don't get too bad.

I know it’s been a couple years since you all last posted here. I just joined and came across your comment. I am 43 years old, male and also have a bladder neck obstruction and was advised by the urologist to get a BNI, but I have held off.
My sleep has been bad for about 3 years. I’ve tried a similar medication to flomax, called alfuzosin and also silodosin, but side effects of congested nose are what bothers me the most so I stopped taking it. I probably didn’t give it long enough.
Wondering if you got your surgery and if so did it help? Would you recommend sticking to the medication to see if it helps more?
Thanks,
Mauricio
