Regarding the internal shaking, mine responded to a dietary supplement from Metagenics called Intesol (antispasmodic GI support). Its main ingredients are chamomile flower extract, peppermint leaf oil, lavender flower oil, and sodium copper chlorophyllin. I am on a regimen being treated with several nutritional supplements in a specified order for hPylori, leaky gut, and yeast, parasites, and bacteria. I was a mess and have made steady progress toward regaining health, weight, and am starting to add a little muscle. I really believe my internal shaking was from the spasms caused by the excess air. I was so full of air caused by the conditions mentioned above. I found GI specialists did not really understand the severity or cause of symptoms. I found it hard to function in daily life. All of this causes or adds to anxiety which only makes all GI symptoms worse. Mine all started with massive antibiotics for about three conditions in a row 2-3 years ago.
Something else I just thought of. The stuck in throat thing I felt when I was full of air. It helped to drink water and force the air down. Also when I took my daily allotment of Gas-X, I found activated charcoal tablets (found over the counter in drug stores or Walmart) to help get rid of gas. But the supplements are helping to get rid of the gas producers.

I know this is way past your post but I had endometriosis years ago
Nobody knew what was wrong until surgery to seee and it was endometriosis.
I have everything that you do including the lump in the throat, kinda like I'm being strangled especially when I lie down. The internal vibrations are the worst for me. I literally can barely function with them. You didn't mention insomniac which was my first clue that something was wrong. But it wasn't long before the lump in the throat and the internal tremors were contributing to my not sleeping. I've been to 6 Drs in 5 months for this and I'm suppose to see a neurologist next but they haven't even called to refer me yet. Might I add that I've lost faith in most Drs since all this started for me. Seems nowadays if they can't relate to what you are saying they just dismiss you as crazy. Truth be told this condition, whatever it is has made me not want to live anymore if I have to live with the internal vibrations. Mine are worst in my chest, stomach and basically my whole trunk. I'm also experiencing massive muscle spasms in my back and my arms and hands along with all this insanity. Has your sleep been affected? Hoping you are on the mend.
Oh my gosh, I'm so sorry you're going thru all of this and that those doctors have been so invalidating!!! I know that feeling of everything coming back normal.
I'm also experiencing the internal shaking that's relieved by gas/bowel movements and am wondering if it's MS/Young-Onset Parkinsons (I posted a similar query here which got some helpful responses: https://connect.mayoclinic.org/discussion/finger-twitching/). I haven't exactly experienced your other symptoms but similar--parasthesia in my arms when lying down, muscle twitching, feeling of something stuck in my throat. The internal shaking is the main thing. I'm curious about the stuck-in-throat feeling and the possibility that it might be linked since you're experiencing it too.
I'm seeing a neurologist in a month and getting an MRI tomorrow and will update you as soon as I can.
I'm NOT a doctor but I've read that Vitamin B-12 deficiency exhibits similar symptoms and was thinking of getting tested for that. Just tossing that idea out there. Hope you find some relief with the gynecologist
