My doctor tried to order one at that pharmacy and they knew nothing about it. Thanks for all the helpful answers to my last questions. I just need to know where to get one.

@helem When you say "my doctor", is this doctor a pulmonologist? I'm seeing a pulmonologist and got one from him directly from his office after an appointment during which he thought I should use one. Of course I had to pay the co-pay part of the cost.
