If you're considering focal therapy, a genomic risk test like Decipher, Prolaris, or ArteraAI would be far more important than the exact T2 stage.

I was a candidate for focal therapy with a single (unilateral) 3+4 (20% pattern 4) lesion, but my Decipher score came in very high at 0.86 and that took focal therapy off the table. I opted for RALP and have recovered well.

Also worth noting that some well respected oncologists do not view focal therapy as curative, but rather a more "active" version of active surveillance. This is UCSF's position, for example. Here's a 15-minute presentation from a conference earlier this year that's well worth watching:

@notpetecrowarmstrong Hello, I am interested in how you are doing after RALP? How long ago, and how are you now? How is your continence and erections? I am scheduled for my RARP consultation this week, and have met with my RO. Both are supposed to be good options. Your history is much like mine. I am leaning towards RARP but am not sure at 68 years old. Any thoughts you could share with me would be appreciated.
