Welcome to Mayo Connect. To request an appointment at any of the Mayo locations, you can either request one for your daughter (self-refer) or ask your doctor to submit a physician referral.
For US residents, if you would like to self-refer, start by completing the intake form or by calling your preferred Mayo Clinic location. Both can be found here: http://mayocl.in/1mtmR63
When filling out the form, you will be asked questions about your concern and insurance coverage (optional). These are short-answer questions to schedule a phone appointment with the scheduler. When you speak with the scheduler, you will have the opportunity to provide more information and elaborate on your answers. You will also be provided information on how to send medical records to Mayo Clinic.
