The GUIDE Model (Guiding an Improved Dementia Experience) is an 8-year nationwide Medicare initiative by the Centers for Medicare & Medicaid Services. It provides coordinated care, 24/7 support, and respite services for people with dementia and their unpaid family caregivers at no extra out-of-pocket cost.

Care Navigation: A dedicated professional helps coordinate medical and community resources.24/7 Support Line: Round-the-clock access to urgent clinical help and advice.Respite Care: Funding up to roughly $2,500–$2,625 per year for eligible participants to give caregivers a temporary break.Training and Education: Classes and skill-building sessions for family care partners.Who is Eligible?Enrolled in Traditional Original Medicare (Parts A and B as primary insurance; Medicare Advantage is generally excluded).Diagnosed with a form of dementia.Living at home or in an independent/assisted living setting (not residing in a nursing home or enrolled in hospice).
