Thank you for responding. I am scheduled for HoLEP. I stopped Flomax because of that feeling. I am hoping it won’t feel the same as that. I’m a pharmacist and know that Flomax affects the smooth muscle tissue and the ejaculation process itself. Relaxing the bladder neck and the prostate. It feels “muted,” less forceful, or less satisfying. Like it stops as soon as it starts. The contraction and whole body orgasm feeling is gone. However, after HoLEP the nerves responsible for orgasm are usually not affected by the surgery but, the bladder neck is altered (causing retrograde ejaculation) but not relaxed like the Flomax . I read that many men say they still experience the same pleasurable muscular contractions of orgasm—they just don’t see semen come out. Have you used Flomax prior and experienced this? Do you still feel the contraction of a satisfying orgasm even though no semen comes out? I’m thinking to try aquablation instead. I’m so worried. But I know I need to do something because I develop bladder stones and have slight irreversible damage to my bladder wall. HoLEP is a one and done procedure. Aquablation is newer and may need to be treated again. They only have 5 years clinical studies.

@alfonse I don’t feel the contraction you mention, but it’s an intense feeling, the muscles in my whole body tighten! I enjoy the sensation for 10-15 seconds. If I would have gone with complete prostate removal as my first Urologist recommended, I fear that I wouldn’t be enjoy my sex life as much as I do now! I have no regrets with HOLEP. In all fairness though, I have no experience with any other procedure!
