BIRADS 5 AND 155.0 cc prostate

New here and thankful to connect. Im 74 years old, scheduled for a targeted biopsy next Friday. Heres my details so far.

🚩Birads 5 parametric MRI finding
🚩Lesion location peripheral zone
🚩 lesion Size .5cm x 1.7 cm
🚩Shape: irregular/ elongated with long curvilinear contact
🚩DWI/ADC: 5.5 water molecule
🚩DCE positive blood flow angiogenisis
🚩 very large prostate 155.0 cc
🚩PSA 17 (adjusted for finasteride)* been on finasteride for 7 yrs

So- going to get this thing examined and scored. Id like to hear any insights particularly from individuals with a prostate the size of mine who opted for surgery. I understand that robotic surgery would be an option?

@jeffmarc Thank you !!!! I’ll look into that for sure. Do you know if he takes insurance? I’m on Medicare and a supplement

My understanding is Medicare insurance will not cover it.

The one big advantage of using this option is that you can talk to the doctor for quite a while about what’s going on with the results of the biopsy.

@jeffmarc thanks ! I contacted them a message to get back to me ! Thanks again !! 500$ is well worth it!!! I can’t get past that my decipher score and my Gleason score are so different!! My urologist likes technology and the decipher score over pathology report

@copyman Thank you so much for your response and the info shared. Yes-PIRADS 5. The Biopsy is scheduled for next Friday. Do I need to request that the Decipher test be run on the biopsy tissue samples? Or is that done if prostate is removed? My urologist did state a PSMA PET would be scheduled as well. Thx again for taking the time to reply. Helluva learning curve right now!

@jeffmarc Info only (sample of one, me). I filled out the online form for Dr Epstein review and never heard back from them (nor did I get any confirmation email so possibly I did something wrong. I could/should have called them (xxx-760-2037) as well but did not so that's on my for not following up.

@floridaman I do not think they take insurance, it's out of pocket and paid 'up front' from my understanding. In your case (4+3) it might be worth it - if he comes back with 3+3 that would be a good change. My biopsy was 4+4 so there would be less chance of a big change for me.

@d3darrow has anyone heard of a 4+3 pathologist report changed to a 3+4 or even 3+3 by another pathologist ? My curiosity is based on my decipher was .34 (low risk) one core only with cancer, clear PSMA scan

I have definitely heard about this happening, Many times.

I do attend nine online advanced prostate cancer meetings every month
