Elevated Liver Enzymes When Taking Exemestane

Posted by mogoodwin @mogoodwin, 2 days ago

I have been taking Exemestane every other day since January 26, 2026. 3/31/26 my Alkaline Phosphatase spiked over 900. Onco ordering CT-Scan and more in-depth bloodwork.

Anyone experience this who is taking Exemestane or otherwise?

Not taking that drug and not to 900... In my case, AST ALT at 75 and 70 but still concerning. I have had extentive blood work plus abdominal ultrasound and fibroscan. Historically I have had high cholesterol and AST ALT near the upper limits but this is a spike which cannot be accounted for by fatty liver, etc. I referred myself to a hepatologist who has ordered more blood work. If these enzymes have gone down, great! If worse with no cause revealed, I may need a liver biopsy. Since this spike, I have stopped all meds and supplements except HRT. I also made a timeline of various changes to meds and supplements. What has your doctor suggested for you?

More in-depth bloodwork and CT-Scan and referred to gastro specialist.

Been on exemestane for a little over a year and no problems with liver values, all still in the normal range. However, I have had increased kidney values last 2 samples starting about 7 months into the exemestane. Just watching for now and seems stable and even a little reduced. However, may have to switch to letrozole soon as I can’t go without sleep and that’s a major side effect for me. Will have to see if all normalizes once off the exemestane.

@mistymar I had a decrease in kidney function while taking exemestane but doctor doesn't seem concerned about that. Maybe it's because exemestane is more tolerable than Anastrazole so I need to stay on it, despite having high cholesterol and some weight gain.

I need to stay on it for 7 more years, which seems like forever.

Initially my Alk Phos was in the too high range and now it's considered too low, I've been on exemestane for 3 years. Both my liver and kidney values are always way off since being on the treatments for my breast cancer. As lesliek mentioned, my dr never seems to care, even when the values seem very bad. They only ever care when my ANC goes to 0.8 or lower, nothing else ever gets mentioned. My PCP gets concerned about all of it but oncology never cares.

@tillymack More in-depth bloodwork and CT-Scan. Ultrasound had nothing startling.
