I have stage 4 metastatic with metastasis to abdominal and chest lymph nodes. Great news, I just finished 2 yrs adt with psa remaining at 0 the entire time! I'm 65 and no worse for the wear with the exception of severe fatigue. I've decided to take a break. Do any of you have ideas to help remedy this fatigue? Do you think a break is a good idea?

After two years, it’s a good time to take a break and see if your PSA stays down. For many people that will stay down for years for Summit comes back quicker than others. To get a PSA test every three months at the very least.

The fatigue will subside over time as your testosterone rises. You could get injections of testosterone, but that could be a real mistake because it could lead to your cancer coming back very quickly. Have your blood tested For testosterone level every time you get a PSA test so you see what’s going on. How quickly it rises, depends on what ADT drug you were on. If it’s Lupron it could take a year, If you were on Orgovyx within six months you would have a decent amount of testosterone and the fatigue would reduce greatly.

One thing that may seem counterproductive is exercise. Doing a lot of exercises is one of the best ways to minimize the fatigue.

I went through the same two year procedure and my risk was high but had not metastisized.
The only thing I might suggest is to not taper too fast from the prednisone you were probably taking with Zytiga. I ached for weeks in my shoulders and hands and was told I probably tapered too fast after being on a low dose (5mg) for 2years.
Best of All to you on your continuing journey.

The other thing about not tapering prednisone long enough is that you will get a lot of fatigue. If You don’t taper long enough youwill almost definitely need to get sleep in the afternoon, You end up exhausted. If you do sleep Make sure you don’t sleep more than about a half hour or you can mess up your nightly sleep. You don’t wanna sleep long enough to get into REM sleep.
