PSA jump from 1.5 to 8.5 in 12 months

Hello all,
My husband's PSA had a significant jump from 1.5 to 8.5 in 12 months. He was also diagnosed this week with Pre diabetes and is in very good shape -not overweight at all.
He was diagnosed with stage 0 skin cancer a few years ago and brother died 4 years ago from very aggressive cancer. They assumed it was skin cancer but by time it was diagnosed had spread and he died 8 months later. Important to note that the brother was also in very good shape and diagnosed as well with pre diabetes and was put on Metformin.
We have an appointment with Urological Oncologist on Thursday and of course very concerned.
Has anyone else seen a similar situation and if so any advice/information to share.

High PHI at 53.7.

We went to the urologist today and got the following results:

My husband's PSA actually dropped from 8.5 to 3.37. It's still a significant rise from 1.5 12 months ago. Most concerning is the PHI is 53.7.

Can anyone else please shed light on similar readings you may have had. The MRI is scheduled for 2 weeks.

We went to the urologist today and got the following results:

Also the DRE was normal.

BPH can cause what’s called happening. The PSA may not return as quickly to normal as you would like, but it seems to be getting there.

It can’t hurt to do an MRI, Though they are so noisy, they’re really annoying.

Was he put on an antibiotic? That can cause the PSA to drop a lot if there is BPH or an infection.

He was not put on antibiotics and all urinalysis tests, etc came back normal.

Most concerning is the new PHI score of 53.7. This test according to our PA is much more accurate than standard PSA test.

From what I have read, 53.7 sits at the upper end of the “high” range, very close to “very high.”

This generally means:

There is a substantial likelihood of prostate cancer if a biopsy is done
Increased concern for clinically significant (more aggressive) cancer, not just low-risk disease. a

MRI is scheduled in the next 10 days so now we have the wonderful waiting game!

@houstonringing Look, it’s gut wrenching for sure but just do what has to be done, one thing at a time, OK?
MRI is scheduled so those results will shed some light on what is going on. It will dictate your next steps…
PSA came down a lot, so that rise could have been due to strain, stress or whatever, so that’s a good thing.
And the PHI index is like the 4K Score - I had one of those and it was completely wrong! My cancer was found on biopsy to be extensive and of a higher Gleason score, and a friend-who also had the 4K test - had minimal cancer, yet his score was TRIPLE mine…so don’t freak out over these different tests.
If the MRI shows anything suspicious, he’ll then have a biopsy (transperineal hopefully) and only then will you know what you are dealing with.
For all of us on the forum, this is life as we know it and it’s no longer a big deal, but for you and your mate it’s a real smack in the head because it all comes at you fast and furious and there’s no way to be prepared for it.

@heavyphil - Thanks for getting back to me. You are right, all we can do at this point is simply get it done. We will know soon enough what the next steps are. We are very lucky to be part of the Northwestern Medical Group in Chicago. Our specialist does transperineal MRI assisted biopsies if one is needed.
I appreciate all the comments and will keep the group updated when we learn more after the MRI.
