@charlesprestridge - Is it common for the patient to request that the tumor tissue be tested for the Decipher score? I'm wondering why this hasn't been suggested or even mentioned.
I had an RP 16 months ago, and am soon to have IMRT. PSA is at 0.07, but with close to 3 month doubling, (0.02, 0.04, 0.07). I have a Gleason 4+5, EPE, SVI, pT3b, pN1. And 8/27 lymph nodes with tumor. Seems like I'm an ideal candidate, or is it too obvious I'm in deep do-do.
I am now questioning if ADT with ARI along with the IMRT regimen is required if it's less aggressive. Often the ADT is delayed as it can cause hormone resistance.

@krs03 To rest your mind it’s been proven that early adoption of ADT does not cause castrate resistance; in fact, it reduces the number of cells which can spread, mutate and then go on to be resistant.
ADT is a key component in your treatment, especially if you have an aggressive, fast growing type.

A decipher score is based on tissue, not on blood, So they would supply tissue. If you had a decipher score, they would send samples of your tissue to have the test done.

My surgeon asked if I wanted to have the Decipher test done on my Biopsy. I told him yes. He arranged for the Decipher test to be performed.

If your Urologist or Radiologist does not ask you about the Decipher test, then it is perfect for the patient to request the test.

City of Hope, where I am going has a very good patient portal. I ask questions between visits and to prepare for upcoming visits. Hopefully, you have access to this feature. The Urologist and Radiologist and staffs have been fantastic. I will submit questions to nurses and to the doctors.

I had a followup MRI last Friday, to prepare for a biopsy this Friday. Initial results of MRI were posted to portal on Sunday. I did not expect to hear anything from Surgeon prior to biopsy. However, he sent me a quick message yesterday that the MRI did not change from last year and he would see me on Friday. I was amazed he took the time, in a high-volume facility to send me the message.
