I experienced similar symptoms for a number of years along with a numb left foot.
I assume you have a MRI to indicate l3-S1 nerve compression.
It sounds like your tried everything.
The only thing that worked for me was to reduce inflammation in the back as much as possible.
Maybe surgery is needed.

@bad56 May I ask what you have done that actually reduced the inflammation? I have tried so many things but stay in constant pain.
