How are things since 2023 when you didn't do ADT?
I am leaning not to take either.

Your situation sounds similar to mine. AS since 2012 with low psa scores until they rose to 6.2 last year over a six month period. MRI showed One small lesion about 10 millimeter.

Targeted biopsy showed Gleason 8. Elected to have 5 session proton sbrt. Oncologist wanted ADT for 6-18 months. My understanding is that ADT during radiation helps a lot, and not so much after the radiation. I chose to do 5 months of Firmagon. Side effects were minimal other than reducing testosterone to near zero and loss of libido. Firmagon has a half life of about 55 days I think? So I will be “on ADT” for about four more months before Firmagon is mostly gone. Current psa is < .02 or undetectable.

If I had it to do over again, I would do the exact same thing. My opinion is that the two of you should consider short term ADT.
