PROSTATE AND SEMINAL VESICLES, RADICAL PROSTATECTOMY: - PROSTATIC ADENOCARCINOMA, GLEASON GRADE 3+4 = 7 (GRADE GROUP 2) - CARCINOMA INVOLVES BOTH SIDES OF PROSTATE, AND APPROXIMATELY 15% OF PROSTATIC TISSUE - NON-FOCAL EXTRAPROSTATIC EXTENSION BY CARCINOMA IS IDENTIFIED - BILATERAL SEMINAL VESICLES ARE FREE OF CARCINOMA.
Then I “think” they took 7 lymph nodes and they were negative?
LYMPH NODES, LEFT PELVIC, DISSECTION: FOUR LYMPH NODES, NEGATIVE FOR METASTATIC CARCINOMA (0/4)

LYMPH NODES, RIGHT PELVIC, DISSECTION: - THREE LYMPH NODES, NEGATIVE FOR METASTATIC CARCINOMA (0/3)

While you have a 3+4 the non-focal EPE Is a serious problem. It would almost be mandatory to be on ADT with this. I am sure your doctor will talk to you about it.

Non-focal extraprostatic extension (EPE) in prostate cancer means cancer has spread extensively beyond the prostate capsule into surrounding fatty tissue, typically covering more than one or two microscopic sections (often defined as >1 high-power field). It signifies a more aggressive tumor, higher risk of biochemical recurrence (BCR), and poorer outcomes than focal EPE.

At this point, I would want to ask the doctor were the margins clear. I don’t suppose they are because EPE says it has spread extensively beyond the prostate. You need to speak to the doctor about recommendations for the near future.

@mcnite yes it is a bit over whelming. Lots of new medical terminology...but essentially this:

you have a somewhat aggressive Gleason 7 cancer...but many have more aggressive and lived a long time...you will be given ADT therapy ( prob Orgovyx-one a day..first month is tough usually getting used to side effects+ another 2nd gen drug prob)

PSA will usually drop dramatically in first 3 months w/ ORGOVYX !

after 3 months you will prob get 9 weeks of radiation ( 45 sessions)

Lots to learn- start exercise program, start eating healthy foods, educate yourself and make sure your docs talk to you and tell you what is happening...welcome to the club...lots of info on this site..but make sure your questions are coherent...you will be rewarded..we are on your side.. good luck !
