Does Pluvicto work?

Hi,

Docetaxel doesn't seem to be working..My husband's PSA is up now to 500!! He's had 2 treatments of Docetaxel so far, but they are only giving him 1/3 dose, because he had a tumor removed at his spinal cord at T4 in November and is trying to get mobile now.

So, does Pluvicto work? Is it as good as Docetaxel? What is the prognosis with Pluvicto? This is all so scary.

Some people have some major side effects from Pluvicto, most don’t. I have heard at least 5 people talk about their experience, In some cases they had to have more time between doses because of the side effects. The guy that leads the reluctant brotherhood advanced prostate cancer forum Had three doses of Pluvicto and it brought his PSA down to .05. He’s holding off the other three for a rise in PSA. He was a real success story.

I saw a webinar where they discussed a person who lit up the Pet scan With metastasis from head to Legs. He had one Pluvicto Treatment and it almost cleared his pet scan, But his side effects were so bad that he could not take any more treatments

Be aware that it works really well for 33% of people OK for 33% of people and not at all for 33% of people.
If you have certain genetic issues, either hereditary or somatic, it can affect how well Pluvicto Works. You can ask for a somatic test before doing Pluvicto To find out if you’ve got Genetic changes Due to the cancer.

If you have BRCA2 or ATM It seems to work better. If you have RB1, PTEN or TP53 They are Pluvicto resistant.

Pluvicto works. After 25 years of living with cancer, my psa and psma scans show no evidence of cancer. Treatments 1 through 5 were with relatively low side effects , i probably should have delayed the 6th and have let the side effects improve. Bottom line ,cancer free with minimal side effects. I am one of the fortunate ones. No chemo, immunology provonge, xtandi, and of course 25 years of hormone injections.

I have had PC since 2003 PSA 13.9 Gleason 3+4 when I had seed implant, 40 direct beam radiation, and 3 years of zolodex. The 17 years of remission. In 2021 my PSA shot up to 36 and it was back and in bones and lymph nodes. I had 4 years Lupron and erleada then last year PSA started going up again slowly. I have had three shots of Pluvicto and have noticed some symptom improvement but my PSA continues to go up. It is 6.6 now and slowly but steadily increasing since I started Pluvicto when PSA was 2.8. Is Pluvicto a failure? What should I do?

A good friend of mine, now 82, has been on Pluvicto for over a year. He says it’s the best he’s felt in over four years. The only negative is he has to isolate for a week due to the radioactivity. Hope this helps. Good luck.

Gleason level? Did you have your prostate removed? What was your initial PSA level? Were there metastases? What's your age? Any genetic mutations? Thank you.

That's strange. I also thought they'd prescribe Plutovicto, but I didn't have surgery, I had chemotherapy with docetaxel + Zoladex 10.8, and now I'm on ADT + Erleada (Gleason 4+5, PSA dropped from 530 to 0.03). Do you think it makes sense to take Plutovicto for me? I have three bone metastases and multiple lymph node metastases, but my doctor said they're is sleeping.

I'm 84, I inial psa 15 Gleason score 4+3 ,prostate removed in 2001. 6 months later psa up to 5, put on ADT still on it. Xtandi last 7 years kept psa under control. Psma scan showed metastasis in lymph nodes. Did the 6 pluvicto treatments, it worked for me. No genetic mutations. If you're psa is that low and Dr says they're dormant, I would wait until they become active. Also by waiting there are better and newer options in the pipeline. Good luck.

You say your surgery was in 2001. Six months later, your PSA went up to 5. You’ve been on ADT since 2001 or 2002? Did you become castrate resistant and that’s why they put you on Xtandi?

Must admit, being on ADT for over 24 years is pretty incredible. I’ve been on an eight years and that’s longer than other people I’ve heard from. I attend 9 advanced prostate cancer meetings every month, that length of time is unusual.

I was always worried that being on it for this long would make me get neuroendocrine prostate cancer.

I've been on ADT since 2001. They put me on xtandi in 2017 after psa increased, it kept my psa less than 1.0 for 8 years. Before xtandi I had immunotherapy, provance, which didn't do anything. Then zytiga which lowered psa but drove up my liver functions. Then Pluvicto, starting April 2025, 6 treatments ending in Nov 2025. All the time on ADT.
