Lingering abnormal lab work
I had chemo in 2024, 4 times. My latest white count dropped 1000 points to 3000 in a 5 month period. There are a few little abnormalities in the differential. No Dr. seems concerned, but I am. I asked to have a recheck, which will be done this week. I was 84 at time of chemo. Is my bone marrow too old to recover and what is the reason for these changes? I hope to get answers. Am waiting for an appointment regarding low gamma globulin too. This whole breast cancer situation is quite the adventure.

A few things I have learned in watching my WBC over the years, including differential, because mine tends to run low all of the time.
It is important to look at the trend, rather than just the absolute numbers. So you are right to pay attention to a noticeable drop.
My numbers vary a lot, depending on the time of day of the blood draw. My WBC is lowest in the AM when fasting, much higher later in the afternoon on a full stomach. My oncologist told me that this pattern is due to cortisol fluctuations in the blood. Cortisol tends to be high first thing in the AM. Are you comparing the WBC at the same time of day?
Finally, I have found that yes, the oncologists don't pay much attention to the WBC. If you want someone to help you gain a deeper understanding of your immunity, you will probably need to see an integrative doctor or funcional medicine doctor. You will have to pay out of pocket. Many are MD's or nurse practitioners that formerly worked in "mainstream" medicine. They are willing to run more in-depth lab testing on your WBC, including mapping levels of T-killer cells and other subsets of WBC.

2 ReactionsIf your WBC was always around 4,000 and now is 3,000 that is a shift but not necessarily concerning. Like olderbreastcancer, my WBC is always low, sometimes out of range, but that is normal for me. Your WBC is still within normal range and small variations outside the set parameters in the differential are common and meaningless, I always have them, too. My training is in Medical Laboratory Science so I am familiar with these tests. Still, your doctors should have explained why they weren’t concerned and it’s fine to ask for a recheck. Do you otherwise feel well? No viral infections recently?

2 ReactionsI had 4 rounds chemo late 2023 followed by 4 weeks radiation. My white count was running low after finish with a low lymphocyte count as well. He always said “bloodwork looks good”. After the third blood count like this I questioned it and was told it’s normal and would watch it. White count barely got up into normal 2 months later but the low lymph’s continued. Asked again couple months months later and he said it can take 2-3 years for lymph count to become normal and sometimes it never does. Just means I’m mildly immunocompromised. So I monitor it for changes but otherwise as long as stable, not considered really abnormal. White count was low again last time after being normal for a year but we just monitor and wait.

I will have follow up imaging for some abnormality that showed up six months after lumpectomy I am nervous about what it could be

3 ReactionsOncologies need to pay attention to Cancer patient's lymphocyte count drops - under 1000 were alive 4 years after surgery compared to 23% with lymphocyte counts above 1000. There are new studies indicating that survival depends on your NK cells - they need to be turned on - often cancer treatments will suppress these natural cancer killer cells, also older patients will have lower counts making them even more vulnerable. Your doctors need to check ALC levels...lymphopenia is a immune disorder that exists. There is a drug approved for specific bladder cancers called ANKTIVA a immunotherapy drug that acts like a boost for your body's natural killer (NK) cells-the immune system's "assassins" that detect and destroy abnormal cells like cancer. In cancer patients, NK cells often become suppressed, reduced in numbers. Unfortunately the FDA has not approved this for breast cancers (yet) but many are actively pushing for this therapy to be available to all cancer patients - they are seeing a direct link to survival when the body's lymphocyte count is boosted. As a side note - my breast cancer returned after 30 years in my lymph nodes - could be my age at 72 or covid but something jump started it. My lymphocytes were low at 1400 but after surgery, radiation dropped further to just under 1000 - declined chemo but staying on AI and taking ZOMETA infusions every 6 month to prevent cancer from moving into my bones. My last lymphocyte count was slightly increased to 1100 which I find encouraging - trying to manage with diet, exercise, sleep. I am pushing my oncologist to watch my white blood count and keep me informed of any need immunotherapy like ANKTIVA approval for breast cancers. I am convince this therapy separately or in conjunction with other anti cancer drugs could be a life saver.

6 ReactionsI have leukemia and now diagnosed with invasive lobular carsonoma breast cancer. My counts are all over the place. I see an immunity MD also on top of my onocologist/hemotologist for treatments. He has added an immunity infusion called Gammagard. It is keeping my infections at bay.

1 ReactionI had my follow up appointment and also repeat labs. I went in the afternoon since my morning got crazy on me. The result was interesting. My WBC went from 3000 to 5000 and my Hgb and Hct dropped. Opposite of my morning lab work results. This fits in with the comments that lab work varies with the time of day. Next lab work I will go back to mornings so a more accurate trend will be present. Still no Dr. is worried about it. I have decided not to be worried either but I will insist my lab work be followed more than once a year.
