@tw508 I believe my pulmonologist got it approved because I also have ABPA. I don't currently have Aspergillus but I did when I was hospitalized. I think my eosinophil levels were initially high. She did do the prior authorization for me, I'm still on it. 300mg (3 shots at a time) once a month. My Dr at NJH wants me to stay on it, so I keep doing it.

@hbcupcake Thanks very much. We did not get approved by insurance, but still working at getting it. High eosinophils, high IgE, but must also fail a treatment of long acting bronchodilator and steroid puffer. A ridiculous requirement, given history, but what can one do but play the game.
