Last January (2025) at 72 years old a prostate biopsy showed that I had a significant 4+3 tumor on the right side and a smaller 4+3 tumor on the left side. In February I had a PSMA pet scan that showed no cancer outside of the prostate. My highest psa had been 10.3. Due to riskier 4+3 cancer and its large size I chose RP which was completed on May 5. Surgery revealed that I had a very large prostrate almost 3 times as large (83g) and that it protruded up into the neck of the bladder. So as part of the surgery the surgeon had to reconstruct the neck of my bladder adding about an hour to the surgery time. Overall the surgery went fine even with this difficulty. The post surgery biopsy of the prostrate showed that I did not have clear margin on the right side and that the cancer had actually spread outside the prostrate to both of the seminal vesicals. The cancer was also identified as cribiform and intraductal. Surgeon was able to save the nerve on the left side but not on the right side and the seminal vesicles are removed as part of the RP. As a result of these biopsy results I was rated a high risk and pTb3.

Post surgery recovery followed the normal path but I had to wear the catheter for 14 days due to the bladder reconstruction.

Ok, current condition 9 months post surgery. I had very little incontinence from the very beginning and so am mostly back to normal. ED is still here and doing penal rehab with daily 5mg cialis and vacuum pump so still hoping for the best. Use Bimix injections (Trimix too painful) with good results. Good news is that 4 PSA tests post surgery all show no detectable PSA so I am very hopeful for the future. So with the results showing significant cancer that had spread outside of the prostrate (which was not evident in the PSMA pet scan probably due to their very close relationship with the prostrate) and the nature of the cancer I am very happy having had the RP. Not happy with the ED but that is part of the process and recovery, but I am alive and right now cancer free. Hope this helps. Rick
