L-spine Pain

Help in understanding of why no relief from Physiatry, and my pain from PT, cant get my pain under control.
Degenerative changes at L5-S1 with impingement of the bilateral S1 nerve roots
at the lateral recesses. L4/L5: Disc desiccation. Concentric fissure left posterolateral annular fibers.
Moderate bilateral facet arthropathy. Mild ligamenta flava hypertrophy. No
spinal or foraminal stenosis.
L5/S1: Moderate disc bulge with impingement of the bilateral S1 nerve roots at
the lateral recesses. Moderate facet arthropathy. Mild-to-moderate bilateral
neural foraminal narrowing. No spinal stenosis. ANY input on what I can do to achieve a pain free life, and work a normal 40hrs a wk. Thanks in advace.

Can you describe your pain? mine is sometimes alon the lower back above the top of the pelvic bone. Mostly it's in a spot to the right of the spine and at the top of the pelvic bone. Kind of a burning sensation. I don't believe that there are any major nerves in the area. The worst is that when I start walking muscles in my lower back start to tighten up and I feel like I'm getting bent over and need to stop. None of the things that are supposed to help with nerve pain seem to offer any relief. I had this pain before and after my L5-L1 laminectomy procedure.

Can you describe your pain? mine is sometimes alon the lower back above the top of the pelvic bone. Mostly it's in a spot to the right of the spine and at the top of the pelvic bone. Kind of a burning sensation. I don't believe that there are any major nerves in the area. The worst is that when I start walking muscles in my lower back start to tighten up and I feel like I'm getting bent over and need to stop. None of the things that are supposed to help with nerve pain seem to offer any relief. I had this pain before and after my L5-L1 laminectomy procedure.

Mine is left posterior pressure starting @ butt Crack, goes into left leg/ hip tingling, burning, clenching, weakness, along w/ left lower back into muscles in left butt cheek/ to foot, and left LQauad pain. Sitting is the worst, then laying on back, and I am unable to turn on the left, pain is unbearable. PT has told me my pelvis is not symmetrical and they have adjusted and I do get relief for 1 or 2 days, then it goes right back, to the severe pain. As long as I do not sit at all, and am upright, no bending, the pain just radiates into my legs then feet, of course cuz when I sit, cant get back up.. im unsure if I can take this pain much longer and have a very high tolerance for pain, and do not take medications.. I wish doc's could figure out so I can go back to my very active physical life..

Agreed with your statement, "The worst is that when I start walking muscles in my lower back start to tighten up and I feel like I'm getting bent over and need to stop", is exactly how my left side is...

Do you possibly have glutineal tendinopathy that was discussed here earlier? FWIW I don't have any pai in the glutes or down either leg.

We have spoke a couple years back which then was my C-Spine, and either its Still C-Spine. Severe feeling in l-spine/ lower back is horrendous. Im working on getting back to Mayo, and praying they let me see Dr. Fogel.

I have similar problems with LHS impinged nerve at L5/S1. I posted earlier at "Any suggested alternatives to an ALIF at L5-S1?"
My pain is in the left glute, left lateral quad, left lateral calf muscle and tingling and loss of feeling in left foot. I was informed my my neurosurgeon the an ALIF was ultimately my solution, but I am going for another opinion in 3 weeks' time to another neurosurgeon.
I believe a foraminotomy (with laminectomy) is the least invasive surgery to resolve this.
The issue is that a pinched nerve under these circumstances seldom resolves on its own - some intervention is often (not always) required.
I've run out of attempts at facet injections, epidurals, nerve root sleeve blocks and rhizotomies.
