Decipher for Subsequent Biopsies
I'm scheduled for my second annual biopsy next week. I had the first one Decipher-scored. Should I get Deciphered for this one as well? Is a Decipher score likely to change (possible to change)?

I am so sorry to read your story as we all want to hear the words “we caught it early” and have pathology confirm that. What was your PSA leading up to the diagnosis? Also what was your MRI Pirads score?
My PSA was 6.1 at the time of diagnosis. I did not have the MRI. I had the biopsy and the PET scan showing prostate-confined disease...which it wasn't...it had already broken through the membranous capsule and spread to a small degree into my left (only) seminal vesicle. Decipher Score was 0.5, showing low probability of metastasis and mortality. Between 5 and 15 years, I have a 93% - 97% chance of still being alive. I am 70 years old.

Mine got me out of ADT, not by the Decipher score, but by details in the grid.

1 ReactionEvidence indicates your Decipher score can change from a second biopsy from that found in a first biopsy; but as of 2021 (date of this paper) the jury’s is still out as to how significant any observed change actually is regarding true progression….maybe more recent studies provide more definitive information.

“In a man on active surveillance for low-risk prostate cancer, a doubling of the Decipher biopsy score within genomic low-risk category from first to the second biopsy related to biopsy reclassification to Gleason grade group 4 on the third biopsy. However, the final pathology at radical prostatectomy showed Gleason grade group 2 with an organ-confined disease. This case suggests that the genomic risk category of Decipher biopsy scores during active surveillance may be more informative than either the interval genomic score change or the biopsy Gleason grade group.”

CONCLUSIONS: Among patients undergoing Decipher GC testing during AS for prostate cancer, there was substantial variation in serial test results; however, increases in GC were not associated with short-term biopsy upgrade.

Seems serial Decipher testing is of limited value….at least based on these two studies..

Thanks for checking! Waiting for the biopsy results to confer with my doctor.
Biopsy was today and went OK. Lidocaine for the jabs; MRI-guided. Prep caused more discomfort than the actual sampling. In fact, because my prostate is so large, I suggested that the urologist take some extra samples. He said, "I think you're the first patient who ever asked me to do that." But he didn't take extras. When the results come back, we'll have the Decipher discussion.
