Testicular pain about prostatectomy: Any advice?

Posted by golfermike @golfermike, Feb 27, 2025

Gentlemen,
I had a robotic prostatectomy four weeks ago. One week ago I began to have significant testicular pain. Odd, since I did not experience this the first three weeks after surgery. Testicles are tender and get progressively more sore during day, making it more painful to move around. There is no swelling, bruising, internal bleeding or fever to note. Advice or recommendations are welcome. Thank you. Golfer Mike

How well have you stuck to the "light duties... no lifting" recommendations?
I did pretty much nothing for the bulk of the 6 weeks post-op.

Thanks for the response. Overdoing it at work is possible. I will try to slow it down. I am probably am putting in 30 office hours a week at this point. Maybe need to back off?

I had my surgery in September & I still haven't worked a full day.

My timeframe was longer than 4 weeks. It was about 6 months. Mostly in the right testicle. I had some pain into the abdomen. Turns out I had epididymitis. Its an inflammation of the cord leading to the testicle. It took about 3 weeks of Cipro.

I has pain in my left testicle within days of my robotic prostatectomy. The pain continued and after 2 months they did an ultrasound which showed the testicle had shrunk a little but showed no other problems. My testosterone levels also fell so I went on HRT. The pain was worse at night when laying in bed on my left side. After another 12 months of pain they did another ultrasound and found the testicle had atrophied and was down to 50% of its original size. Also the testicle had lost its form and was now soft and "squishy" like a marshmallow. I was told the testicle was now non-functional, so 3 months later I had an orchiectomy to have it removed.

Interesting. I started developing testicular tenderness about 6 months after radiation. I still have my prostate but was doing Kegels to try to lessen dribbles after peeing. Stopped doing the Kegels when the dribbles decreased and I read about pelvic floor tightness being a possible cause of the tenderness. My urologist confirmed that last week and is giving me a referral to a pelvic floor specialist. The point being to keep the floor strong but to make it more flexible. He did examine my testicles and said there was some atrophy but normal for someone on Orgovyx and Xtandai. BTW I am Gleason 8 metastatic low volume.
