I had HoLep in 2022. I thought mine was very large at 210gr. I was told by multiple urologists that I had one of the fastest growing they had seen. I see that the largest recorded is about 700gr and the largest for HoLEP was 696gr. The surgeon that did Urolift in 2014 and Greenlight in 2019, both a waste of time (from my perspective) referred me to another surgeon to do RASP. That surgeon said he thought I would be a good candidate for HoLEP and I liked that HoLEP has no incisions and has the highest rate of success. My HoLEP surgeon was very skilled and I have had no issues. That was in San Diego. Whichever procedure you go with, I would research to make sure the surgeon is highly skilled and has performed a high number of procedures. Looking at the pros and cons for a 580gr prostate both procedures are good choices. RASP would go faster with Robotic assistance where HoLEP would be a longer procedure but an experienced surgeon should handle successfully. I was still working when I had the surgery and the fact that I would be back to work in a few days factored in my decision to go with HoLEP. Whichever procedure you have I wish you success.
