@leslieb52 every two years but reading some of the other comments, I see that some are able to get one every year .
Talk to your doctor as I am going to talk to mine about getting a DEXA test every year. I am due for one in June, so I am hoping the scores are better and it shows the Prolia is working.

This is @slr1 I am going to pay for a bone density dexa text after 1 year cause insurance only covers one test ever 2 years. If the dexa in 1 year shows my t-scores are worse I will get on Prolia.
