Investigate Tulsa procedure. Surgery may be over treatment at this point. I have a form of buyers remorse after 4 years following surgery. Maybe I didn t do enough research. Quality of life is important as well as longevity.

@billfarm I wholeheartedly agree. My young oncologists seemed insistent on my planning to live to 95, when I'm already 74 and will be pretty satisfied just to reach 80 without prostate cancer being the cause of death. Due to the PSMA PET showing spread to nearby lymph nodes ("oligometastatic?"), I've had 44 radiation treatments and just completed a year on Orgovyx with another 6-12 months planned. PSA and Testosterone have been < .01 since May, but life has greatly changed with regard to energy levels, the romantic relationship between my wife and me, and a host of other side effects. I'm grateful that the disease is controlled, but it's come at considerable quality of life cost. Working full-time would be out of the question and I'm completely exhausted at some point every day. I'm hoping by next Christmas to be at least somewhat recovered, but past 70 means Testosterone recovery is a coin toss. No surgery, but I feel your pain otherwise.
