Why continuing pain three years after a successful hip replacement?

My wife had a hip replacement three years ago. The surgery was, by all measures, successful. Yet it became painful starting after three months and the pain has gotten worse. All kinds of treatments have been tried, including physical therapy, steroid injections. The implant has been examined by the surgeon and by other orthopedists. No one can find the cause of the pain. A Mayo post by Liza Torborg has suggested three possible causes: infection; a loose ball in the socket;; and psao tendinitis . The first two have been ruled out. What specialist at Mayo Clinic can diagnose the third possibility?

I believe you need to look for an appointment in orthopedics. If conservative measures to address psoas (iliopsoas) tendinitis have failed, further imaging and diagnosis may be required. There is a surgical procedure to release an overly tight tendon, but I think other causes must be ruled out first.

I will see Dr. Hevesi on May 5 for what I believe is iliopsoas tendinitis/tendinopathy. What is the surgical treatment? I have done physical therapy and injection along with trying exercises and swimming. Nothing has helped and most increase the pain.

I found this site when Googling: 'hip pain after hip replacement,' I had the surgery two and a half years ago and all was well until three nights ago. I deliberately tried to sleep only on my right side, because I have a painful frozen shoulder on the left side. I did put weight on my tight hip when asleep. When I woke up, I could hardly walk , because my entire right leg hurt so much. It is better now but walking is painful and I limp badly, as a result. I am terrified I will have to have another surgery due to a loose ball in the socket. I'm 83, so any surgery is risky. Christmas is ruined and I don't know how soon I can get an appointment. Thank you.

I had the exact same pain years following my hip replacement in 2021, I had several steroid injections and another MRI to make sure the components where all in the proper place and they were, it turns out I have iliopsoas bursitis and it’s very tight , I had the release procedure done in December 2024 at Mayo by Dr Mario Hevisi , I think I spelled the last name wrong but so far it feels a little better I am doing PT three times per week to try and strengthen my hip/ legs and stretch the muscles I’m hoping in a few more months I will get more positive results .

Hello, i am doing better some days better than others. I do believe as well as my doctor and physical therapist goes I'm probably at the best i will get. i know how to mange my hip pain through meds exercise and stretching. The cortisone injections in my hip do not do much at all. My only regret is not going to Mayo clinic from day one.

I have similar. THR post 4 1/2 years. Joint on MRI is perfect but have been in pain for 4 years. I've had shock treatment, cortisone injections and loads of physio. Suspicion is on either the Tensor Facia Latae, the Glutes Min or and the abductors. I cannot walk without pain and serious pain after 3-5 minutes. Sleep on the affected side very difficult. Next short term suggestion, Botox Jabs and the Pulse Radio Therapy all at £1100.00 a pop!!!

@frankcat have you had a Cobalt / Chroumin test done? Have you had an MRI and nothing wrong with a second opinion! My wife has had 6 hip surgeries; 14 years later, hopefully no more surgeries for years to come!

Hello, yes i did have a couple MRIs done no one mentioned anything about a cobalt/Chroumin test done. Is that something they have to look out for or a special test? or will that show up during a normal MRI? the only thing that shows up is the Iliopsoas tendonitis/Bursitis and because of all the surgeries i have atrophy in the muscle. That is what i am trying to strengthen now with PT. I will bring up the Cobalt test with my doctor.

@frankcat these test are blood test! My wife’s orthopedic surgeon declined to do test, so I had her primary order it and her levels were toxic. It happens when two medals react against each other, creating this poison within one’s body; from the implant having two different types of medal. ( titanium in Chrome)
