What % of men after radiation treatment completely lose ejaculation?
This doesn’t seem to be a topic that has been fully discussed. I think it is kind of useful for some.

Neither my Oncologist nor my Urologist told me I would lose my ability to ejaculate after my radiation. It is basically like having your man card taken away. My erections are also not what they were but I don't take anything since the anejaculation makes me unwilling to engage in sex anyway. Before my diagnosis and treatment I was extremely sexually active. Now I may as well be living in a cave. I think people should be aware of those conditions going in. I wasn't.
I received SBRT (9/2024) and remain on ADT (planned 12-18 mos) for Gleason 9 (4+5) localized disease. I was placed on tamsulosin for urinary retention during RT which I have continued, knowing that it caused retrograde ejaculation for me when used previously for BPH (which resulted in my discontinuing the medication then). I was warned that after RT I would have dry ejaculation with or without Tamsulosin. ADT suppresses my libido and makes erections more difficult to attain and orgasm more difficult to achieve. I was warned about all of these issues by both the urologist and radiation oncologist when deciding on treatment options. I have no regrets; my PSA is down from 15.3 ng/ml prior to treatment to 0.18 ng/ml only 10 weeks after radiation was completed. The sexual side effects are frustrating but with the help of my spouse I am able to enjoy our intimacy even without penetrative sex. I hope when ADT is completed that my cancer remains controlled and my sex life improves though I do not expect it to be the same as it was before.

1 Reaction@mouser26 After radical prostatectomy my climax changed. I didn't have ejaculate which "primed the pump" but I have learned to be aware of the arousal is in the mind. I can still climax but the feeling is more in the pelvic muscles. Yes it is different but still satisfying. I don't have any erections since my surgery in July 2025 but it is SLOWLY getting better. I have been will likely take up to a year or more to get erections again but I"ve adapted in my mind to accept the new norm and if it improves over the next year, even better... My .04 PSA after prostatectomy suggests this will be a treatable condition, not a death sentence, in the future.....
