Welcome to Mayo Connect. It sounds like you have already contacted Mayo about an appointment. When you contacted Mayo, did you share your insurance information?
I would recommend contacting Patient Account Services to ask your specific questions.

Welcome @luckydog1, You mentioned the VA doesn't want to refer you to Mayo due to your normal tilt table tests but your CCN specialists have already referred you to Mayo. Since you need VA approval along with the Community Care Network approval for the Mayo referral, can you ask the CCN specialists if they can help you get approval for the referral to Mayo?
