You mention that you are HR+; however, you don't mention if you are ER+ which is the case 80 percent of the time. I am a physician and my wife has breast cancer. I would suggest that you could consider talking one aromatase inhibitor at the minimum. She does take an oral bisphosphonate in addition. I don't believe the side effects are significant and there could be some considerable benefit.
My wife doesn't have much side effects on the one that she is currently takes. She takes Exemestane. She had switched from another one. She takes the oral monthly oral bisphosphonate, Risidronate. An oral bisphosphonate prevents osteoporosis but also reduces the risk of bone metastasis. She has taken the two drugs for 3 years. She wasn't advised to do chemo. She was advised to do RT. She did have a positive node.
I wish the best for you. I have seen very elderly patients in the their 90s with breast cancer have a biopsy to diagnose breast cancer; they wouldn't wish to do surgery or chemo but still they would take an aromatase inhibitor. I believe they are well tolerated. Taking a medication to strengthen the bones would be well tolerated as well and reduce the risk of metastatic breast cancer. Who wants to take a medication. I really wish the best for you.

@eileendover I totally agree with your posting. I was on Anastrozole, switch to Letrozole and doing so much better re: side effects. I will also look into Risidronate to prevent osteoporosis (I already have osteopenia in my left hip) and to reduce the risk of bone metastasis. I also was not advised to do chemo (ineffective) and my oncotype score was 14. I had a bilateral mastectomy, cancer in both breasts, stage IIa. Grade 2 - 12 lymph nodes removed, 1 positive.
I am 70 years young and, frankly, want to do everything I can to avoid a reoccurrence and stay alive. My family still needs me and I am the backbone. So, I am "following orders" - first for myself, and second, for my family.
Imessenger1 - I respect your decision and position but I am a bit afraid for you. All of us are rolling the dice in fighting breast cancer. There are no guarantees on anything...in BC treatment or in life - we have all lived long enough to know this is true. We are living this fact.
I truly wish you the very best as you move forward in life. And hope you find "like-minded" women, like yourself, who are living a full, long life with your/their decision and who will certainly support you. Blessings.

@eileendover " Yes," my cancer is 95% estrogen based. I thank you very much for your suggested AI information, and will look into these specific drugs further.
