Any experiences with HIFU localized ablation for prostate cancer?

I am interested in anyone who has had this treatment for well-localized cancer in the prostate and what their experience was/is.

I had HIFU in 2018 in Charlotte, NC. It was localized. Gleason 3+3, I think. No problems before and none after. PSA went from 4.5 to 0.5. It has inched back up to 1.5, which my uro doctor says is way low. I’m 77. Carolina Urology 704/660-3322 Dr. David Konstandt.

Hello, I had Hi-Fu surgery at University of Chicago medicine. Hi-Fu treats the prostate from the outside of the prostate . Tulsa Pro was offered to me about one and a half years later as a new attempt to reach areas missed from the initial Hi-Fu. Tulsa pro enters the prostate and treats it from the inside out. My argument was even the areas treated with Hi-Fu showed up as returning cancer in the PSMA scan that I had done at Northwestern Hospital in Chicago. That is why I questioned the whole Hi-Fu procedures effectiveness. It just didn't work for me. On a case by case it may work for others. I chose it knowing I only had about a 60% chance ( Dr. quote) of success, as it seemed to be the easiest recovery and least invasive. Ultimately had to undergo SBRT for 27-8 visits along with a hated 4 mth ADT shot. Good Luck to you....

sorry about the relapse. when did you get the IFU at UC? do you remember your #'s pre -op? Did you have a decipher test pre-op?

I HIFU in 2020 and like you PSA went down very low, but creeping back up. PSMA showed PC also spread to seminal vessel. Went to MDAnderson who said they didn’t do HIFU. They did transrectal biopsy without success. Said check psa in 6 months when it more than doubled. They then did transperinial biopsy (which Mayo I read does from beginning) which found Gleason of 9. I had Proton Radiation and Lupron. PSA staying around .03.

My husband has Gleason 8 localized and we have spoke to the Radiation Doctor and the Surgeon. The RD told us about the process and we have a follow-up meeting with him this week. Last weeks spoke to the Surgeon and he mentioned Surgery or HIFU. That was a little confusing to me because HIFU has not come to play in my research. My husband liked the idea of HIFU instead of surgery/radiation. It is a strong possibility his insurance will not pay for HIFU. However, I have requested a second opinion with another Surgeon at a different institute to get their options. My husband is OK with a second option because if he goes with either options it want happen until the new year. Any thoughts?

I have a friend who has three 4+4 = 8 lesions and just had HIFU done in CT. I warned him that for Gleeson 8's ChatGPT 5 says there is up to a 50% PC recurrence rate. Chat recommended ADT + 25 IMRT sessions + HDR brachytherapy for his clinical profile.

Despite the Chat GPT info...he said he just wanted "one and done". He told me he has to have another MRI in two weeks " to see if they got it all". I am praying for his long term remission

@kikito1 HIFU has been around for over 20 years. More urologists and Rad Interventionists are offering it in the right cases. Find a HIFU expert and make an appointment. There are other focal therapy modalities available. It all boils down to your staging, tumor location, your individual goals, and your consent to try this option with its issues over reoccurrence. Most insurance pays today, but many HIFU doctors need cash up front because insurance/medicare programs are stingy and not paying enough to the doctor.
