Tymlos blood pressure side effects etc. very nervous! Help…

I wanted to know if anyone has some info on these specific topics.
We are in a small town one Rheumatologist…has connections with Dartmouth, which isn’t bad.

1. 90/60 blood pressure (10-20% drop with Tymlos)
2. Can you start with smaller dosing first few days to start? He says no others say yes. Tymlos company rep says no, but I see others have said they did that.
3. Can you use Ibuprofen to raise low blood pressure?
4. Phenegran or Zofran for Nausea? What other meds since these can low BP also.
5. Calcium Vitamin supplement schedule?
6. How soon should a Blood test follow up be?

Any other experience with Tymlos would be helpful…supposed to start Thursday

I started having increased palpitations (I've had them for 30 years, no big deal) within a couple weeks, but the symptom I needed to address was exhaustion and weakness. I just couldn't keep up with my life, and felt so weak. Cutting back to 60mg did the trick (I still get lots of palpitations, they are harmless PVCs). My lumbar spine T-score was down to -3.6 and the doctor was quite concerned with that. I had just finished 5 years of risedronate, and although my hip score had improved, my spine had deteriorated considerably.
Getting this diagnosis with the warning "don't lift anything more than 20 lbs, don't fall, etc", has changed my life, in that I feel so fragile and am afraid of doing things I loved, like skating, skiing, even bicycling. I'm so afraid of falling, that I have changed my outlook on life activities, and not for the better. It's a bit depressing!

I wonder if you have anything else going on? Good doctors seem to be much harder to find these days. Ones that are passionately learning new info all the time because they actually love what they do…helping people.
Blood test preceded with lots of important questions.
Blood work…anemia, Vitamin D …not that Tymlos might not cause those symptoms….there are so many other things possible too that could be the cause …unrecognized major stress from all this…certainly has caused similar symptoms…
Also I don’t know but I read a few studies showing improvement happened 18-24 months and continued after moving to another drug…usually a bisphosanate…Evenity is different and has been coming up more and more.
For some even though both Forteo and Tymlos are anabolic they work slightly differently….im hoping this continues to go as well as the last 14 hours since my first shot.

darling husband,
she's past the worst but could be unsteady for a bit. If she gets a headache caffeine and something salty should take it away. Bless you both.

I had my husband give me the injection at around 7:30 pm last night.
For blood pressure took two ibuprofen to elevate my BP just a bit but it helped from 60mm to 65mm diastolic for me. Enough to make a difference. My heart rate increased about 20bpm but after about an hour came down within my normal baseline of 65-68 …it did alternate to 74 for a while of and on…probably just anxious…my husbands heart rate is normally around 60-65 was 85-88! It didn’t come down for a couple hours.

Who is delivering the injection today?
While I'm glad you took the 80mcg without effect, I'm reassured that the nurse was approving a lower first dose. Well done, you brave soul.

Pharmaceutical companies do not just magically decide on a dosage level. In the early stage of development, dosage level had to be varied in order to decide on the "optimum" dosage for the average person. There is a movement in the cancer field to rethink this. (I cannot think of the name for this program now.) That quality of life is also important and should be balanced with effectiveness. I have mentioned this Shkreli Award winner before.

"3. Amgen. Amgen’s cancer drug Lumakras, approved in 2021 at a 960 mg daily dose, showed in trials that a lower 240 mg dose offers similar efficacy with fewer side effects. Despite this, Amgen continues to market the higher dose and seeks final FDA approval at that level, potentially protecting $180,000 in annual revenue per patient. Pharmacologist Donald Harvey, PharmD, interviewed by KFF Health News, called Lumakras “a poster child for incredibly bad development.” Amgen defends the 960 mg dose, citing FDA acceptance of it as the recommended level. "

You absolutely can start with lower dosage of Tymlos . I did 20 for 2 weeks, 40 for 2 weeks , 60 for 3 weeks, than 80 full speed ahead. It’s been a year now with 20+ bone increase in the spine much less in femur.

@vixstermoves Amazing! 🙌🏻 Well done! Thank you for your reply. I’m finding that anecdotally, women who titrate UP (as opposed to starting at the full dose and titrate down when they experience uncomfortable side effects), seem to get to the full dose comfortably, stay on Tymlos LONGER (which is the goal), and have those positive gains.

In the same vein, many also see positive gains in under the 2 years it’s recommended.

One rheumatologist told me to start low.

Another said absolutely not. This is the doctor I prefer to go with, because her practice isn’t nearly as chaotic as the first guy, but I honestly feel I might do best starting low and slow.
