You have certainly been through an ordeal – I feel for you!
Fortunately, when my left rod failed, it exited through the uretha – only 2-3mm inside the tip – so no damage to the glans. As I was travelling in a poorer country, I decided to remove the rod myself on the spot (5-10 cm was already protruding) and then went to emergency mainly for antibiotics to guard against infection. They also inserted a catheter which (in hindsight) probably wasn't necessary.
My surgeon subsequently offered me three options: (1) Replace the missing rod with another malleable one. His (and my) view was that the same thing could well happen again due to some anatomical reason. (2) Replace the prothesis with an inflatable one. I was not keen on that option due to the greater complexity of the device (with added potential for failure) and the greater cost of the device and associated surgery. (3) Do nothing and see how I fare with the single remaining rod. I chose this option for the time being, partly because I was not keen on undergoing further surgery and the fact that the single rod was somewhat working for me. Some 15 months later – so far, so good!

@tex47 I can see you making that choice after all that and having had 3 penile surgeries in 12 months I can related. I have a friend who had a IPP implant worked for years was installed by my dr. It failed and he did not want to travel back to my area to get it done. So his urologist recommended someone local to him. The dr. pretty much destroyed his penis in the end. He ended up back at my dr to have it removed put one rod in for 6 months to allow things to heal. He had everything taken out and is now very depressed over the whole thing. I'm still upset my dr. won't address the fact that my implant is faulty because it won't bend correctly.
