Boniva and tooth extractions
I took Prolia for 2 years and had good results. Then i had a broken tooth extracted and they refused my next infusion. My dr put me on Boniva in April of this year and i have an infected tooth that needs to be pulled Is that safe or do I need to wait 3 months without the Boniva before the extraction. What can I take in the meantime for my bone loss

Are you taking the pill or IV? The pill carries much less risk.
Is your dentist experienced in managing bisphosphanate patients? If you don’t know, you should ask.
There is something called atraumatic extraction technique. Make sure you have a dentist (preferably an oral surgeon) comfortable and experienced with this technique. It is supposed to decrease osteonecrosis risk. Also I think there are recommendations surrounding which types of anesthesia are best. Anesthetic with vasoconstriction properties (shrinking of the blood vessels) are to be avoided. Sometimes they are necessary anyway because of bleeding risk.
Ask your dentist about antibiotics before and after surgery. To my knowledge this is standard of care.
From what I know a drug holiday from oral meds is generally not necessary. My research isn’t all that recent but from what I recall, it has been studied and not found to be of benefit as bisphophonates stay in your system for years. I’m not sure about IV meds
If you smoke, stop, it increases risk of osteonecrosis
I hope this helps
Thanks that really did help. Will try to find a dentist that is knowable
@laura1970 For 4 years i was taking Boniva pill then the last 2 years Prolia infusion. When i broke my tooth the hospital stopped the Prolia infusion and the dr put me back on Boniva pill. So just taking Boniva since April 7 months
@wsparrotlady since it is the oral pill, to my knowledge you do not have to stop taking it, as it stays in your body for years. That having been said, each doctor has his/her own opinions about current studies. I would follow your doctor’s advice
